Immunization Requirements

Students must download and print the Immunization Medical History Form. Upon request, the Wellness Center will mail or fax an Immunization Health History Form to you.

In order to register, this form must be completed, signed, and returned to:

All students new to USF St. Petersburg are required to submit the following form and immunization documentation prior to being able to register for classes:

USF Immunization Policy # 33-002
An official translation is required for any forms not in the English language.

  • Immunization Medical History Form: According to Florida Administrative Code Rule 6C-6.001(5) “Each student accepted for admissions shall, prior to registration, submit on a form, provided by the institution, a medical history signed by the student.”
  • MMR 1 & MMR 2 vaccines OR titer blood test
    • Provide proof of two MMR vaccine dates (given after 1st birthday) OR a titer blood test showing positive immunity against the Measles & Rubella diseases.

  • Meningitis vaccine OR declination.
    • Menactra/MCV4 vaccine (given after age 16) is an optional vaccine, however, it is strongly recommended. Students can decline the vaccine through OASIS or by checking the box on the Immunization History Form.

  • Hepatitis B vaccine OR quantitative titer blood test OR declination
    • Hepatitis B is an optional vaccine, however, it is strongly recommended. Provide three (3) Hep B vaccine dates OR a quantitative titer blood test showing positive immunity OR decline the vaccine on the Immunization History Form or through OASIS.

  • TB Screening Skin Test OR Blood Test OR Chest X-ray
    • Tuberculosis Screening is required for all international & U.S. Born students residing at an address outside the U.S.
      • Approved Skin Tests: PPD or Mantoux
      • Approved Blood Tests: Quantiferon gold (QFT) or T-Spot

SUBMITTING YOUR FORMS

Upload all documents via the Admissions Portal. To upload, sign in (right side of web page) and select My Workspace, then choose My Documents and upload your forms.

If you are from Florida and your healthcare provider is a participating physician, you can log in to Florida Shots to print a copy of your Immunization Records to submit with your Medical History form.

TEMPORARY MEDICAL EXEMPTIONS

Must be submitted by the attending physician, must include reason for exemption and duration of exemption.
