Fires and Property Damage
In the event of a fire and/or damage to University property, other than automobiles, complete the following form. Please contact Katherine Simpkins (kogelvie@usf.edu) to discuss the process for an insurance claim, if applicable. If there are injuries involved, see the Injury/Illness reporting procedures at left.
