Referral Form
The Care Referral allows a student to self-refer or allows faculty, staff, students, and University of South Florida community members to refer a student of concern to Student Outreach and Support (SOS).
If you are aware a student is experiencing challenges or difficulties, please share the concern using the form below.
Our goal is to intervene and offer support before a student reaches a critical level. This will be reviewed by an SOS staff member during university business hours.
This form is NOT monitored 24 hours a day and does not contact first responders. If this is an emergency please call University Police at 727-873-4444 or 911.
